Use this W-2 form to report wage and withholding information for employers. This 4-part carbonless set of forms for employers includes copies A (federal, red scannable), 1 (state and city) and D (file). A second set of forms for employees includes copies B (federal), 2 (state and city) and C (file). Your typewriter or pin-fed printed will print the employer and employee forms in 2 different passes. This ensures the legibility of all information on all parts. Forms are 8 ½” wide (format changed in 2001). Mail employer copies in a batch to the SSA. Mail employee copies in envelope #DWENV05 or #SWENV05.
