This 4-part carbon-interleaved form includes employer copies A (federal, red scannable) and D (file) and employees copies B (federal) and C (file) which are pre-inserted into an envelope. When you use your typewriter or pin-fed printer, the mailing address will also be printed on the envelope. You just seal and mail! Forms are 8 ½” wide (format changed in 2001). Mail employer copies in a batch to the SSA. Employee copies are ready to mail.
